Advanced Disposal Services, Inc. Information Request Form

Levi & Korsinsky, LLP has commenced an investigation into the fairness of the sale of Advanced Disposal Services, Inc. ("Advanced Disposal" or the "Company") (NYSE: ADSW) to Waste Management, Inc. ("Waste Management "). Under the terms of the merger, Waste Management will acquire Advanced Disposal for $30.30 per share.
